TERMS AND CONDITIONS FOR PRODUCTS AND SERVICES PROVIDED BY PROMIDENTAL.COM
Treatment Plan. Promidental.com prepares surgical templates according to the submitted treatment plan. Promidental.com does not review or check submitted treatment plans. The practitioner holds final responsibility for the treatment plan, and the medical procedure performed on the patient.
Third party services. Promidental.com may use third party services (which comply with their high level of quality standards) to perform part or all of the processes involved in fabricating the surgical template.
Timing. While Promidental.com does everything possible to fabricate the surgical template within 7 business days some extenuating circumstances or problems with equipment or other may cause unexpected delays.
Tubing. Promidental.com does not insert or attach any additional tubing or sleeves into the printed template.
Termination. Promidental.com has the right to refuse service or to discontinue service at any time.
Responsibility. Promidental.com is in no way responsible, financially or otherwise for any complications, injury, discomfort, which may result from implant surgery, drugs, anesthetics or otherwise. The use of the surgical template and its effects on the patient is solely the responsibility of the clinician.
Modifications. This agreement can be modified at any time without prior notice.
LIMITATIONS OF LIABILITY. PROMIDENTAL.COM EXPRESSLY DISCLAIMS ANY RESPONSIBILITY FOR ANY DAMAGE, LOSS OF PROFITS, LOSS OF BUSINESS, LOSS OF DATA, DAMAGE TO COMPUTER OR PERIPHERAL EQUIPMENT, OR ANY OTHER DIRECT, INDIRECT, INCIDENTAL, CONSEQUENTIAL or punitive damages, and all losses of any kind, arising directly or indirectly from the use of PRODUCTS OR SERVICES PROVIDED BY PROMIDETNAL.COM, caused to the USER, patient or any third party.
THE INDIVIDUAL ORDERING PRODUCTS OR SERVICES VIA PROMIDENTAL.COM, HEREBY ACKNOWLEDGES THAT THE AGREEMENT HAS BEEN READ AND UNDERSTOOD, HAS THE AUTHORITY TO ENTER THIS AGREEMENT AND HEREBY AGREES TO THE TERMS AND CONDITIONS OF THIS AGREEMENT.
